For every hotel you can create custom forms.

In the Form details page, the title field is used internally while the Description field appears to your guests.

You can set the language of the form. While this is optional, if you define a language and you have multiple languages enabled (see Hotel Settings for that) you will need to create the same form in all selected languages.

The Days offset field is the number of days before the guest’s departure that the form will appear to the guest (backward offset). For example, if a guest arrives at 19/05/2015 and stays for 4 days (departs at 23rd of May), a form with the Days offset set to 2 will appear on the 21st of May.

There is also the option Forward offset. Enabling this option will make the form to appear so many days after the guest’s arrival. For example, if a guest arrives at 23/05/2015 and stays for 6 days (departs at 29th of May), a form with the Days offset set to 2 will appear on the 25th of May.

The offset settings apply for the review form as well.

On forms other than the main, can manage on the left the fields of the form. You can set the order by dragging between the fields. For each field you can also set its type and if it is required or not. Deleting a field will also delete any data submitted, so it is advised to use the Active checkbox.
